Гострий некротичний панкреатит: сучасні підходи до хірургічного лікування

Abstract

We have analyzed our experience of surgical treatment of 183 patients with acute necrotizing pancreatitis. ln the beginning patients were managed with therapeutic manipulations, at first we have used intravenous infusions of ozonyzed sodium solution. 43 patients (23,50%) underwent video-laparoscopic manipulations in enzyme-toxic period. Transcutaneous interventions with US-control were performed in 141 (77,05%) patients. Open surgical manipulations were performed in 59 (32,24%) patients. In 43 (72,88%) patients purulent-necrotizing pancreatitis was observed. Postoperative mortality made up 10,93%. Surgical tactics with individualization, ozonotherapy, wide usage of miniinvasive manipulations and delayed open surgical manipulations, if needed, made it possible to improve significantly the results of surgical treatment of acute necrotizing pancreatitis.
