Особливості анестезіологічного забезпечення при хірургічному лікуванні в гострому періоді розриву артеріальних аневризм головного мозку (огляд літератури)

Abstract

In the review of the literature the modem representations about anesthesiological maintenance are submitted at operations in the acute term of а rupture of arterial aneurysms of а brain. Is shown, that anesthesiological maintenance at the patients with intracranial aneurysms • multiple problem, which performance demands not only knowledge and skills, which allow to carry out а general anaesthesia, but also and knowledge of neuroanatomy, neurophysiology and neuropathology. The purpose of the anaestheslologist is, that on the terminal of operation the patient should bе in consciousness, slightly sedatition, with normal for him bу а level of arterial pressure. For achievement of this purpose it is necessary to execute some specific problems: prophylaxis of а repeated rupture of an aneurysm up to and during operation, maintenance of surgical access with optimum visualization of а surgery field, support more adequately cerebral perfusion and protection of а brain against а hypoxia during operation, prophylaxis of the deferred ischemia of а brain, maintenance of quiet awakening.
