Значення ендотеліальної дисфункції у післяопераційному рецидиві варикозної хвороби нижніх кінцівок та її корекція

Abstract

У 65 хворих на варикозну хворобу нижніх кінцівок проведено визначення маркерів ендотеліальної дисфункції (ЦЕК – циркулюючих ендотеліальних клітин, sVCAM-1, Р-селектин, Е-селектин, t-PA, ендотелін-1) до виконання операції ендоваскулярної лазерної коагуляції вен (ЕВЛК) у ранньому (10 днів) та пізньому (60 днів) післяопераційному періоді. Встановлено значне підвищення вищеозначених показників до операції ЕВЛК та їх незначне зниження у післяопераційному періоді. У 51 хворого, котрі однакові за стадією, віком та статтю із попередньою групою хворих, з метою корекції ендотеліальної дисфункції, за два тижні до ЕВЛК та впродовж двох місяців після операції були застосовані флеботропні препарати (детралекс та квeрцетин). Нами констатовано, що застосування вищеозначеної комбінації препаратів є ефективним засобом відновлення функціонального стану ендотелію, про що свідчить суттєве зменшення показників ЦЕК та sVCAM-1 у післяопераційному періоді. In 65 patients with varicose disease lower extremities carry out definition of markers endothelial dysfunction (CEK-circulating endothelial cells, sVCAM-1, P-selectin, E-selectin, t-PA, endotelìn-1) to the operation of endovascular laser coagulation veins (EVLK) in early (10 days) and late (60 days) postoperative period. In 51 patients, identical in stage, age and sex from a previous group of patients, with the aim of correction of endothelial dysfunction for two weeks to EVLK and for two months after the operation were applied flebotropi drugs (detralex and quercetinum). Found a considerable increase these indicators to EVLK and a slight decrease in postoperative period.The application combinations of detralex and quercetinum are effective means of restoring the functional condition of endothelium, as evidenced by significantly reducing the index of CEK and VCAM-1 in the postoperative period. The use of flebotropni drugs detralex and quercetinum may be one of the means of prevention of postoperative recurrence of varicose disease, taking into account that they have a positive influence on functional condition of endothelium, leveling the manifestations of endothelial dysfunction.
