Когнітивні порушення у хворих, котрі проходять лікування програмним гемодіалізом

Abstract

З метою виявлення когнітивних порушень у хворих, котрі знаходяться на лікуванні програмним гемодіалізом і уточненням їх частоти та характеру, було проведено дослідження когнітивних функцій з використанням нейропсихологічних тестів: короткої шкали для дослідження психічного статусу, батареї тестів для оцінки лобної дисфункції, тесту малювання годинника, вербального тесту О. Р. Лурія – запам’ятовування 10 слів, таблиць Шульте, а також шкали депресії Бека. У дослідженні взяло участь 30 пацієнтів відділення нефрології та програмного гемодіалізу Закарпатської обласної клінічної лікарні ім. А. Новака: 16 чоловіків (53,4%) та 14 жінок (46,6%). Середній вік хворих, що взяли участь у дослідженні, склав 50,1±7,3 року, 52,4±4,6 року для чоловіків та 47,4±8,2 року для жінок. Дослідження когнітивних порушень при допомозі батареї тестів для визначення лобної дисфункції виявили, що когнітивні порушення реєструються у всіх хворих, що проходять обстеження, у 21 пацієнта (70%) когнітивні порушення відповідали легкому типу, а у 9 пацієнтів (30%) було виявлено деменцію лобного типу. У більшості хворих, які знаходяться на лікуванні програмним діалізом, мають місце когнітивні порушення неальцгеймерівського типу різного ступеня вираженості, які потребують обов’язкового лікування. One of the complications of maintenance hemodialysis is dementia, which primary reason is an insufficiency of cerebral circulation. Well-timed reveal of mild cognitive disorders of vascular genesis in such patients could make possible the prompt launching of preventive treatment and to prevent the heavy complication – dementia. Was the detection of cognitive disorders in patients, which are getting maintenance hemodialysis treatment as well as specification of its rate and characteristics. All together there were 30 patients participating in the research in the unit of nephrology and maintenance hemodialysis of Transcarpathian Regional Clinical Hospital named after A. Novak: 16 men (53,4%) and 14 women (46,6%). The average age of patients taking part in research was 50,1±7,3 years, 52,4±4,6 for men and 47,4±8,2 for women. The following neuropsychological tests were used for the evaluation of cognitive functions: short scale for investigation of psychological status, the set of tests for evaluation of frontal dysfunction, test of clock drawing, verbal test after O.R. Luria – memorizing of 10 words, table after Shulte and the scale of depression after Beck. The study of cognitive disorders with the set of tests for evaluation of frontal dysfunction has revealed that cognitive disorders are being recorded in all patients undergoing the examination; the mild cognitive disorders were found in 21 patients (70%), frontal dementia was detected in 9 patients (30%). The cognitive disorders of non-alzheimer type with different degrees of manifestation are taking place in majority of patients getting maintenance hemodialysis and demand mandatory treatment.
