Класифікація методів хірургічного лікування геморою

Abstract

На основі аналізу особливостей патогенезу гемороїдальної хвороби розробити універсальну патогенетичну класифікацію методів хірургічного лікування геморою. В статті проведений аналіз особливостей патогенезу гемороїдальної хвороби, які відіграють важливу роль у систематизації методів лікування геморою за їх суттєвими ознаками. Узагальнені та виділені основні способи впливу на патогенез гемороїдальної хвороби, які використовуються в сучасних методах хірургічного лікування геморою: ліфтинг (або гемороїдопексія), зниження внутрішньоанального тиску, дезартеризація верхньої прямокишкової артерії, часткова редукція або деструкція кавернозної тканини, повна редукція кавернозної тканини. Зазначені напрямки лікувального впливу покладені в основу класифікації методів хірургічного лікування геморою. Запропонована патогенетична класифікація методів хірургічного лікування геморою передбачає виділення наступних класифікаційних одиниць: категорія, клас та група методів хірургічного лікування. Пропонується виділяти шість класів методів хірургічного лікування геморою: 1) методи дезартеризації верхньої прямокишкової артерії; 2) методи зменшення маси кавернозної тканини; 3) методи зниження внутрішньоанального тиску; 4) методи повної редукції кавернозної тканини (гемороїдектомії), 5) методи ліфтингу (гемороїдопексії) та дезартеризації верхньої прямокишкової артерії; 6) методи комбінованого впливу. В кожному класі методів пропонується виділяти групи методів хірургічного лікування геморою, які мають спільні суттєві риси в методиці виконання, які в значній мірі впливають на результати лікування та структуру можливих ускладнень. To design universal pathogenetic classification of methods of surgical treatment of hemorrhoids on the basis of the analysis of pathogenesis’ peculiarity of hemorrhoidal disease. It has been performed the analysis of the pathogenesis’ peculiarity of hemorrhoidal disease, which acts the important part in systematization of the methods of surgical treatment of hemorrhoids in essence. The main methods of influencing to pathogenesis of hemorrhoidal disease are abstracted and separated, which are using in modern methods of surgical treatment of hemorrhoids: the lifting (or hemorrhoidopexy), the decrease anal pressure, the desarterisation of upper rectal artery, the partial reduction and destruction of cavernous tissues, the full reduction of cavernous tissues. Its directions in treatment of hemorrhoids are laid in the basis of classification of the methods of surgical treatment of hemorrhoids. Proposed pathogenetic classification of methods of surgical treatment of hemorrhoids foresees separating the next classification units: the category, the class and the group of methods of surgical treatment. It have been proposed to separate six classes of methods of surgical treatment hemorrhoids: 1) the methods of desarterisation of upper rectal artery, 2) the methods of decrease of cavernous tissues’ mass, 3) the methods of decrease of anal pressure, 4) the methods of full reduction of cavernous tissues (hemorrhoidectomy), 5) the methods of lifting (hemorrhoidopexy) and desarterisation of upper rectal artery, 6) the methods of combined influence. It has been proposed to separate groups of methods of surgical treatment of hemorrhoids in each class of the methods, which have common essential features in technique of operation. Its features essential influences to results of treatment and structure of possible complications. Key words: hemorrhoids, surgical treatment, classification.
