Діагностика та лікування синдрому Меллорі-Вейсса

Abstract

From 1982 to 2001 yrs 17407 pts were treated with gastrointestinal bleedings and 1128 among them were with Mallory-Weiss syndrome (MWS): 992 males and 136 females in the age 18-82 years. ln these patients we performed 1786 endoscopic examinations with using different methods of endoscopic hemostasis (ЕН) to control hemorrhage in 432 pts. On emergency endoscopy active bleeding was found in 208 unstable-and stable hemostasis -pts. ln patients MWS was associated with other causes of bleeding. Recurrent bleeding observed in mainly during 12 hours after admission. Among different methods of ЕН (electrocoagulation, injection therapy, tissue adhesives, haemostatic clipping) we found high efficiency of mechanic and thermal methods. We proposed the algorithm of endoscopic monitoring, ЕН for prophylaxis ERB and reduced frequency of ERB from 8,7% up to 3.4% and length of stay from 5,8 up to 3,7 days.
