Abstract

The thesis is dedicated to the problem of clinical course, diagnosis and choice of surgical tactics in case of peptic ulcer (PU) complicated simultaneously bу perforation and bleeding. There is proposed а classification of perforated acute-hemorrhagic gastroduodenal ulcers (PAНGDU). There is considered the syndrome of "minor" symptoms of the combination of perforation and bleeding revealing of which calls forth application of additional methods of examination. There is elaborated а rational surgical tactics in PAНGDU cases depending on ulcer localization, the variant of clinical course of the combination of perforation and bleeding, hemorrhage gravity degree, peritonitis symptoms. lmprovement of diagnosis together with introduction of the means of intensive therapy in PAHGDU cases allowed to lower а post-operative lethality bу 4,7.
