Abstract:	The article is dedicated to problems of diagnostic, treatment and preventive measures for spontaneous internal biliar fistulas (SIВF), as infrequent complication of а cholelithiasis. The results of treatment of 73 patients with the described pathology in surgical clinic of Regional hospital of Uzhgorod are shown. АІІ fistulas were diagnosed during operations on cholelitiasis or peptic ulcer. The detection of air in bile ducts at an X-ray examination and "microcyst" at an ultrasound allowed us to suspect а presence of а fistula. Тhе treatment of SIВF is usually an operation that consists of liquidation of а fistula, cholecystectomy with sewing of defect of an internal hollow organ, plastics of extrahepatic bile ducts with recovery of their permeability and outboard temporary drainage.
