Засоби хірургічної корекції ускладнень портальної гіпертензії у хворих на цироз печінки

Abstract

In our study we present results of planned and urgent surgical and endoscopic treatment of 221 patients with hepatocirrhosis (163 men ( 73,8%) and 58 women (26,2%) aged 16-69 years; 83 men (50,9%) and 42 women (72,4%) of them aged under 30years). Emergency cover was provided to 111 of patients with acute oesophageal-gastric bleeding. АІІ patients according to Child-Turcotte-Pugh classification were divided into 3 groups: "А" - 69 patients, "В" - 61 patient, "С" - with decompensated cirrhosis - 91 patients. Vascular shunting operations were performed in 84 cases, in 3І cases -operations of disconnection, in 28 - operations of devascularization, in 97 cases - endoscopic sclerotherapy (in 78 patients as primary method and in 19 - as additional procedure). Constant hemostasis саn bе achieved bу vascular shunting operations only in case of dosed portal decompression with ligation of left gastric vein. Operations of devascularization in 67,8% and operations of disconnection in 65% can provide constant hemostasis in case of oesophageal variceal bleeding in patients with cirrhosis. Endoscopic sclerotherapy helps to arrest bleeding from oesophageal variceal veins in 88%.
