Високі пошкодження і посттравматичні стріктури печінкових проток

Abstract

В роботі запропоновано нову класифікацію пошкоджень позапечінкових жовчних проток і стріктур, які ро- звинулись після пошкоджень. Класифікація включає не тільки анатомічні характеристики пошкодження, але й характер пошкодження (перев’язка, висічення, надсічення), клінічний перебіг (підтікання жовчі, перитоніт, нориця) та результати лікування. Особлива увага приділяється розташуванню ділянки злиття печінкових жовчних проток стосовно до печінкової паренхіми – виділені три типи співвідношення між злиттям та печін- ково-печінководуоденальною складкою. Розроблені чотири принципи відновних операцій на позапечінко- вих жовчовивідних шляхах. Запропонована класифікація та принципи дозволяють оптимізувати хірургічну тактику і досягти хороших результатів у 90-93% з летальністю 2%. The study presents a new classification of extrahepatic bile duct injuries and strictures that developed after injury. The classification includes not only the anatomical characteristics of damage, but also the nature of the injury (ligation, excision, incision), clinical course (bile leakage, peritonitis, fistula) and the results of treatment. Particular attention is paid to the relative position of hepatic duct confluence and the hepatic parenchyma – there are selected three types of relationship between confluence and hepato-hepatoduodenal fold. Four main principles of restorative operations on the extrahepatic biliary tract are developed. The proposed classification and principles allows optimization of the surgical tactics and achievement of good results in 90-93% of patients with 2% mortality.
