Запальні захворювання кишки: нові підходи до лікування

Abstract

У даній статті проведений аналіз різних підходів до лікування запальних захворювань кишки (ЗЗК). Описані переваги і недоліки терапії “Крок вгору” та “Крок вниз”. Вибір підходу до лікування ЗЗК має ґрунтуватися на визначенні фенотипу захворювання, оцінці ризику на момент постановки діагнозу на підставі клінічних, серологічних та генетичних даних. The approach to the management of IBD will be based on accurate and detailed phenotype and risk assessment at diagnosis based on clinical, serologic and genetic profiling. Those patients with a global risk that puts them at higher likelihood of rapidly progressive disease and the development of disabling complications should be treated with the most effective therapy as early as possible (currently anti-TNF alpha agents). May also need to consider combination therapy of infliximab with AZA, this has clearly been demonstrated to be superior in efficacy in the SONIC trial. Patients with a lower risk profile can be managed with a more traditional stepwise approach. We need to use mucosal healing as the key measure of response.
